As a Care Navigator / Health Coach, you will be a primary point of contact for members in our MSK program. Your job is to activate members, sustain participation, remove barriers, and reinforce clinically aligned behaviors that improve function and reduce pain. You’ll blend two strengths: Care Navigation: Managing a caseload of active users by guiding members through the program, coordinating touchpoints, triaging needs, synchronous and asynchronous communication and simplifying the healthcare system. Health Coaching: using behavior change techniques to improve adherence, confidence, and self-management for chronic MSK conditions. This is a high-touch, high-volume, member-facing role requiring empathy, speed, and strong clinical judgment within scope.
