Telehealth lets you meet with your Protera Health care team by secure video, audio, messaging, or other electronic tools instead of in person. Care may be delivered in real time (live video or phone) or asynchronously (for example, by reviewing photos, forms, or messages you send).
This consent covers care provided by Protera Health, Inc., Protera Health Medical Group, PC, and Protera Health Medical Group of Michigan, PC (together, “Protera Health”).
During a telehealth visit, your clinician may review your medical history, discuss your symptoms, observe you by video, and recommend testing or treatment. Because the visit is virtual, your clinician may not perform a full physical exam and will rely on the information you share. If the technology is not working well or a virtual visit is not appropriate, you or your clinician may stop the visit and reschedule, or you may be referred for in‑person care.
Telehealth can save you time and travel and make it easier to access care. Telehealth also has limits and risks, including interruptions or poor connections, and the risk of unauthorized access to electronic information despite reasonable safeguards. If your needs are better met in person, we may ask you to schedule an in‑person evaluation.
By consenting, you allow Protera Health clinicians and care team members to evaluate and treat you via telehealth. This may include medical guidance, wellness advice and counseling, physical therapy or exercise instruction, prescriptions, lab work, imaging, diagnostic tests, and other outpatient procedures, as appropriate and allowed by law. You may decline or withdraw from any part of your treatment at any time.
We use reasonable safeguards to protect your information. Your health information may be documented in our electronic medical record and may be shared, accessed, or transmitted through electronic systems, including third‑party platforms, for scheduling, billing, and clinical care. Members of your Protera Health care team may view your information to support your treatment. With your permission and when required by law, we may share information from your telehealth visit with your primary care clinician or other providers. Telehealth may use third‑party platforms. Other authorized personnel may help operate these platforms, and reasonable steps will be taken to maintain confidentiality. We may contact you through the contact information you provide (including email, SMS/text, or patient portal) about appointments, test results, prescriptions, and follow‑up. Standard messaging and data rates may apply. Do not record telehealth visits unless your clinician agrees in advance.
If appropriate, your clinician may prescribe medication or order labs or imaging. Prescriptions may be routed to a pharmacy you choose. We may work with third‑party pharmacies, laboratories, imaging centers, or other partners to support your care. Use of any medication or service is your choice, and alternatives may be available.
Please share complete and accurate information about your health and medications, use a private space and a reliable connection when possible, and tell your clinician if the technology is not working or you cannot safely continue. Keep your contact and pharmacy information up to date. Tell us where you are located at the time of your visit; laws and clinical standards vary by state, and you must be located in a state where your clinician is authorized to practice.
People respond differently to care. Protera Health does not guarantee that a specific recommendation or treatment will improve your condition. In some cases, symptoms may persist or, rarely, worsen. Please communicate any concerns or side effects to your care team promptly.
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room. For urgent symptoms after a visit, follow your clinician’s instructions or contact your care team.
If you have questions about telehealth, ask your clinician before or during your visit. You may file a complaint with your state medical board regarding telehealth services; contact information is available on request.
