Protera Health is a fast-growing digital health company transforming musculoskeletal (MSK) care for adults with back, neck, and joint pain. We partner with health plans to deliver a modern, virtual-first care model that improves outcomes and reduces avoidable spend—including high-cost surgeries, procedures, imaging, and unnecessary in-person utilization.

We’re hiring a Care Navigator / Health Coach who can build trust quickly, guide members through a structured MSK program, and drive consistent engagement—especially for an older population managing joint and muscle conditions.

The Role

As a Care Navigator / Health Coach, you will be a primary point of contact for members in our MSK program. Your job is to activate members, sustain participation, remove barriers, and reinforce clinically aligned behaviors that improve function and reduce pain.

You’ll blend two strengths:

  • Care Navigation: Managing a caseload of active users by guiding members through the program, coordinating touchpoints, triaging needs, synchronous and asynchronous communication and simplifying the healthcare system.
  • Health Coaching: using behavior change techniques to improve adherence, confidence, and self-management for chronic MSK conditions.

This is a high-touch, high-volume, member-facing role requiring empathy, speed, and strong clinical judgment within scope.

What Success Looks Like

  • Members feel understood, supported, and confident in their care plan, measured by their satisfaction score.
  • Engagement and adherence are high (sessions completed, exercises performed, check-ins maintained).
  • Escalations are appropriate and timely (red flags, worsening symptoms, care gaps).
  • Members make smarter care choices—fewer unnecessary in-person visits, imaging requests, and avoidable specialist/procedural pathways.
  • Documentation is clean, timely, and audit-ready.

Responsibilities

You will:

  • Conduct virtual 1:1 video sessions and asynchronous support (messaging) to coach and navigate members through MSK care.
  • Build curated knowledge program that aligns with their exercise program, focused behavior change, pain coping, nutrition, sleep, and creating sustainable routines aligned to their goals.
  • Use evidence-based coaching skills (motivational interviewing, SMART goals, habit formation) to drive adherence and outcomes.
  • Support older adults with practical problem-solving: technology assistance, pacing, fall-risk awareness, flare management, and confidence building.
  • Identify barriers to participation (fear avoidance, low health literacy, caregiver demands, comorbidities) and address them proactively.
  • Triage and escalate appropriately to clinicians for red flags, worsening symptoms, or out-of-scope concerns.
  • Reinforce the “right care, right time” pathway—helping members avoid unnecessary utilization while ensuring safe escalation when needed.
  • Collaborate in team huddles and contribute to workflow improvements, member education, and quality initiatives.
  • Maintain accurate documentation and meet productivity and quality targets.

  • Operationally sharp: you manage high volume without dropping details; you close loops and reduce friction.
  • A clear communicator: you can explain next steps in plain language and set confident expectations.
  • A safe triager: you know when to reassure, when to coach, and when to escalate.

What Makes a Great Health Coach for Older Adults with MSK Pain

A strong MSK coach for an older population is:

  • Empathic and steady: you can support chronic pain without overpromising; you build hope through progress.
  • Behavior-change skilled: you can move someone from “I can’t” to “I’m doing it,” using small wins and consistent reinforcement.
  • Function-first: you focus on what matters—walking, stairs, sleep, independence, confidence, and fewer flare-ups.
  • Comfortable with complexity: you can coach around arthritis, osteoporosis, diabetes, obesity, anxiety/depression, and medication considerations (within scope).
  • Tech-supportive: you can calmly help members succeed in a virtual model.

Requirements

  • 2+ years experience in health coaching, care navigation, patient engagement, care coordination, or a related patient-facing role.
  • Certification in health/wellness coaching
  • Comfort with high-volume member outreach and structured workflows.
  • Strong motivational interviewing and behavior-change fundamentals (formal training preferred).
  • Excellent documentation, time management, and remote communication skills.
  • Ability to work from a quiet home workspace with reliable high-speed internet.
  • Must reside in the United States.

Preferred (Not Required):

  • NBHWC certification (or NCHEC/CHES)
  • Experience in MSK care, chronic pain coaching, PT/OT support roles, or exercise-based programs
  • Telehealth or digital health startup experience
  • Experience supporting Medicare or older adult populations
